Position Summary:


LPN professionals must be first and foremost qualified and compassionate. The ideal candidate will be experienced in providing diverse basic care to those who need it while remaining patient and calm. It is a demanding job but also very rewarding. The LPN will provide comprehensive assessments of physical, mental and psycho-social needs. The LPN must also adhere to policy and procedures, develop care plans to provide effective outcomes in addition to providing direction of other team members as needed. Guided by the comprehensive knowledge of nursing theory and practice, applies the nursing process and is accountable for the health care services and nursing care of a group of specific residents of UAC. Part time position 20 hours per week flexible schedule.


Position Responsibilities:

  • Assist with initial medical assessments
  • Communicate with LPN & RN to assist in the referrals of youth to appropriate medical specialties
  • Assist with required documentation for prescriptions
  • Assist with follow-up services for UAC programs
  • Administer whisper test, vitals, & vision test
  • Provide support with creating and electronically submitting TARs for each UC as needed and required paperwork.
  • Create and update information for each UC in the UAC Portal electronic records.
  • Properly document each medical service.
  • Work collaboratively with the LPN, RN, Case Manager, Clinician, and Cottage Manger Skills Trainer to provide appropriate services to each youth
  • Responsible for all medical reporting requirements
  • Responsible for documentation preparation
  • Responsible for ordering medical supplies
  • All other duties as assigned

Essential Skills and Experience:


  • Ability to speak, read, and writes English & Spanish fluently, which allows full communication with agency staff and 3rd parties.
  • Working knowledge of computer systems, spreadsheet, and word processing software (Excel and Word).
  • Possess a valid NYS driver license with a driving record acceptable to CHK's insurance carrier and possess the ability to drive agency vehicles to other agency sites and places for training purposes.
  • Pediatric, psychiatric experience preferred
  • Ability to work effectively in a team context

Education, Training, & Experience:


  • Current Valid LPN license required
  • At least 21 years of age
  • Hold current American Red Cross First Aid & CPR certification
  • Associate's Degree Required; Bachelor's Preferred in Nursing
  • Experience working in congregate care facility.
